Patient Rights
All patients seen at Iowa State University Thielen Student Health Center have the right to:
- Choose among providers and change physicians, if available.
- Be treated with respect, consideration and dignity.
- Private, confidential and secure medical records.
- Know about their diagnosis, evaluation and treatment options except when such participation is contraindicated for medical reasons.
- Participate in health care decisions except when the participation is contraindicated for medical reasons.
- Information about fees and payment options.
- Information about services provided and his/her eligibility for those services.
- A copy of his/her medical record and the right to restrict its use.
- Information about after hours service and how to seek emergency care.
- Continuity of health care amongst and/or between TSHC, hometown physicians and consulting physicians.
- Submit grievances and comments regarding any aspect of care, treatment or outcome of services provided by TSHC.
- Information about the credentialing process for TSHC physicians, consultants and TSHC staff.
- Request a second opinion from a physician or mental health professional.
- Access information about the above stated rights.
Patient Responsibilities
All patients seen at Iowa State University Thielen Student Health Center have the responsibility to:
- Treat health care professionals with respect, consideration and dignity.
- Refrain from the use of cell phones when in an exam room.
- Provide complete and accurate information and/or documentation about medical history and current problems.
- Provide complete and accurate information and/or documentation about medical history and current problems.
- Follow all instructions regarding treatment, medication, and physician recommendations. If the individual chooses to refuse treatment or recommendations for medical care, he/she is responsible for the medical consequences.
- Tell his/her health care professional about any change in his/her health.
- Keep appointments or notify TSHC as early as possible if he/she will not be able to keep an appointment.
- Pay for all charges that are not covered by the health fee or by an insurance company.
- Provide necessary information to process insurance claims.
- Follow all posted rules and regulations of ISU and TSHC.
