Immigration to the United States

The following information concerns immigration procedures and physical examination.

Those eligible to use the health center for immigration purposes are ISU students and/or their spouses.

Several appointments with the health center will be necessary for anyone applying for immigration change of status. The applicant can make an appointment by calling 515.294-5801 to speak to the appointment staff. The first appointment will be made for the physical examination by the civil surgeon. There is one Thielen Student Health Center physician classified as a civil surgeon by the USCIS.

At this appointment a physical examination will be performed, blood taken for laboratory tests and tuberculosis skin test or chest x-ray will be done.

A second visit will be scheduled with the international travel/immigration nurse. This appointment will be at least one week after the physical examination. The applicant should bring any immunization records he/she may have to this appointment. The applicant MUST bring their passport to this appointment. Failure to bring his/her passport to this appointment means the applicant must reschedule for a later date.

At this appointment the nurse will review immunization records and tell the applicant what, if any, immunizations are needed. With the applicants consent needed immunizations will be given.

A third visit by the applicant will be to sign his/her USCIS papers and have the papers handed to the applicant to be mailed.
