The Notice of Privacy Practices for all Iowa State University Health Care Providers is provided through this link. This notice describes how medical information about you may be used and disclosed and how you can get access to this information.

You will have the opportunity to review and obtain a copy of this notice no later than the first time you visit one of the ISU health care providers listed above. We will also ask you to acknowledge that you that you have been offered this opportunity.

Information Regarding Privacy of Information (HIPAA)

All patients must sign Authorization to Disclose Health Care Information before any protected health information can be released to anyone including parents and/or guardians. This form must be signed before a medical provider or nurse can discuss a student's health status with a parent and/or guardian.   

This form cannot legally be signed prior to an appointment or visit with a medical provider or nurse. It can only be signed at the time of an appointment or for appointments that have already occured in the past.  Consent cannot be given for appointments that have not yet occurred.
